经食道超声心动图缩流直径估测二尖瓣狭窄程度

目的 评价用经食管超声缩流直径估测二尖瓣狭窄程度的价值。方法 对40例行左右心导管检查的风心二尖瓣狭窄患者测量舒张期经二尖瓣口缩流直径(vena contracta),于0°、45°、90°和135°扫描切面分分别有24例(60％)、31例(78％)、35例(88%)和37例(93%)能确定缩流直径。结果 135°扫描切面(左室长轴观)所测缩流直径与格林公式测定的二尖瓣瓣口面积(MVA)的相关性最佳(r=0.83),缩流直径≤1.0cm预测重度二尖瓣狭窄的敏感性为93％,特异性为71％,准确性为86％。结论 采用测定缩流直径来估价二尖瓣狭窄程度,该方法简便,成为其它测定MVA方法的补充。

The vena contracta width of mitral stenosis jet in the assessment of mitral stenosis severity by multiplane transesophageal echocardiography

Purpose To assess the mitral stenosis seversity using the vena contracta width of mitral stenosis jet by multiplane TEE. Methods The vena contracta width of mitral stenosis jet by TEE color flow mapping was compared with MVA calculated by Gorlin s formula. The vena contracta width was determined in 37 of the 40 patients(93%)using the 135 - degree,24patients(60%)in 0 - degree,31patients(68% )in 45 - degree and 35 patients (88 %) in 90 - degree on imaging plane. Results There was a close correlation between the vena contracta width and MVA by Gorlin's for-mula(r = 0.83)in 135 - degree imaging plane.The vena contracta width of color flow mapping^ 1.0cm identfied severe mitral stenosis with a sensi-tiyity of 93 % , specificity of 71 % and predictive accreacy of 86 % . Conclusions Careful color flow mapping of the vena contracta of mitral stenosis jet is convenient and feasible, and would be supplemented to Ither methods in the assessment of mitral stenosis.
