直肠超声引导经会阴穿刺活检术的临床应用

目的 :探讨经直肠超声 (TRUS)介导经会阴穿刺活检术在前列腺癌筛查及低位盆腔肿瘤活检中的临床意义。方法 :采用TRUS介导从会阴经皮肤 ,对 82例PSA >4ng/ml患者行前列腺穿刺活检 ,对 3例PSA持续升高者重复穿刺 ;对 5例低位盆腔肿物患者行穿刺活检。结果 :82例前列腺穿刺活检中检出前列腺癌 18例(2 2 % ) ,重复穿刺 3例中发现前列腺癌 1例 ,5例低位盆腔肿物活检 ,2例为纤维肉瘤 ,1例为何杰金氏病 ,2例为炎性组织。本组感染病例为 1/ 82 (1.2 % )。结论 :TRUS介导经会阴穿刺活检有助于提高活检阳性率 ,且并发症发生率低 ,对穿刺阴性后的可疑病例可重复穿刺

CLINICAL SIGNIFICANCE OF TRANSRECTAL ULTRASOUND-GUIDED BIOPSY THROUGH THE PERINEUM

Objective:To investigate the clinical significance of transrectal ultrasound (TRUS)-guided biopsy through the perineum in detecting prostate cancer and low-pelvic cavity tumors.Methods:Eighty-two patients whose PSA >4ng/ml suspected prostate carcinoma and 5 patients whose ultrasound detecting low_pelvic cavity tumors underwent transrectal ultrasound_guided biopsy through the skin from perineum,and 3 patients whose initial biopsy was negative but subsequent PSA levels were continuously abonrmal underwent repeated biopsies.Results:Prostate cancer was detected in 18(22%) of the 82 patients with prostate biopsy.Prostate cancer was detected in one of the 3 cases who underwent repeated biopsies.The biopsy in 5 patients with low-pelvic cavity tumor showed that 2 were fibrosarcoma,1 was Hodgkin's disease,and 2 were inflammatory tissues.The infection rate of this group 1.2%.Conclusions:Transrectal ultrasound_guided biopsy through the skin from perineum can increase the positivity rate of biopsy in detecting prostate cancer and low_pelvic cavity tumors,and it has few complications.In the patients with continuous high PSA levels,if the first biopsy is negative,rebiopsy should be taken.
