腹腔镜疝囊高位结扎术在儿童腹股疝中的应用

目的:探讨腹腔镜在治疗儿童腹股沟疝的应用价值。方法:收集2011年6月-2012年12月间在我院行腹腔镜疝囊高位结扎术的6岁以下的儿童共360例。其中术前诊断为双侧斜疝36例,术前诊断单侧疝而术中诊断双侧斜疝82例。观察手术时间、术后合并症、平均住院时间和近期随访结果。结果:单侧疝手术时间(11.5±6)min,双侧疝手术时间平均(15.4±3)min,术后24h出院,随访6~12个月,术后疼痛轻微,2例复发,无阴囊血肿、积液、感染等并发症。结论:腹腔镜疝囊高位结扎术治疗儿童腹股沟疝疗效肯定,伤口小,安全快速,操作简便。

Application of the laparoscopic high ligation of hernial sac in treating inguinal hernia in children

Objective： To evaluate the advantages oftransumbilical laparoscopic surgery for inguinal hernias in children. Methods： From June 2011 to December 2012,a series of 360 juvenile patients （≤ 6 years old） with inguinal hernia treated with the laparoscopic high ligation of hernial sac were enrolled. Before operation, bilateral inguinal hernia was diagnosed in 36 cases. During the operation, 82 patients, who were diagnosed with unilateral indirect inguinal direct hernia, were confirmed as having bilateral indirect inguinal hernia. We analyzed the operating time, postoperative complication, hospital stay after surgery and the follow-up. Results： The mean operation time for the single side was （11.5 ± 6） minutes （242 cases）, and that for bilateral operation was （15.4±3） minutes （118 cases）. All patients were followed up for 6 to 12 months. All patients had slight postoperative pain. Two of them had recurrence during the period. No hematoma, scrotal hydrocele and infection occurred. Conclusion： The laparoscopic high ligation of hernial sac is safe, effective and simple for inguinal hernia in children.
