天津地区IL-1 Ra基因内含子2可变串联重复序列与脓毒症的相关性研究

摘    要:目的:探讨白细胞介素-1受体拮抗剂(IL-1 Ra)基因第2内含子中可变串联重复序列(VNTR)多态性与脓毒症的相关关系。方法:应用PCR法,观察90例脓毒症患者、120例健康对照者的IL-1 Ra基因VNTR多态性的分布。结果:两组人群中共发现了IL-1 RNI/I、IL-1RN Ⅰ/Ⅱ、IL- 1RNI/Ⅳ3种基因型,以I型等位基因为主,未发现Ⅲ型和Ⅴ型。正常对照组中Ⅱ型等位基因和Ⅰ/Ⅱ基因型的分布频率与脓毒症组相比明显增加(8%vs2%,P〈0.05;17%vs4%,P〈0.05)。另外,不携带Ⅱ型等位基因的基因型(Ⅰ/Ⅰ和Ⅰ/Ⅳ)和非Ⅱ型等位基因携带者(Ⅰ,Ⅳ)与携带Ⅱ型等位基因的基因型(Ⅰ/Ⅱ)和Ⅱ型等位基因携带者相比发生脓毒症的相对危险度分别为4.3和4.0。结论:I-1RN Ⅱ型等位基因可能在脓毒症的发生发展过程中具有潜在的保护作用。

Abstract:Objective: To investigate the association of sepsis and variable number of tandem repeats (VNTR) polymorphism in the second intron of IL-1Ra gene. Methods: The distribution of VNTR polymorphism in IL-1Ra gene was observed by the method of PCR in 90 patients of sepsis and 120 healthy controls. Results: Three kinds genotypes, IL-1RN I / I ,IL-1RN I/II and IL-1RN I /IV were found in case group and healthy control group, and type- I allele were the mainly.The frequency of the type H alleles and the I / II genoty- pe in the normal control group was significantly increased compared with that in the sepsis group (0.08vs0.02, P〈0.05; 0.17 vs 0.04, P〈0.05), also, the relative risk of occurring sepsis between genotype ( I / I, I/IV) and ( I/II ) was 4.3 and the relative risk of acurring sepsis between ( I / IV) and ( II ) was 4.0. Conclusion: IL-1RN II alleles may have the potentially protective effect in the development of sepsis.
