慢性化脓性中耳乳突炎的微生物学及药敏分析

目的]分析近两年本地区慢性化脓性中耳乳突炎的病原菌的分布和耐药情况,为合理使用抗菌素提供实验依据。方法]取慢性化脓性中耳乳突炎的脓液进行细菌和真菌培养,并对结果进行分析。结果]146耳中培养有微生物生长142耳,其中:细菌培养146耳(培养出150例菌株,混合感染10耳,均为两种致病菌,无细菌生长6耳);真菌培养10耳(培养出6株白假丝酵母菌,无真菌生长4耳),细菌检出率依次为:金黄色葡萄菌球57株(34.34%),铜绿假单胞菌35株(21.08%),变形杆菌15株(9.03%),大肠埃希菌9株(5.42%),肺炎克雷伯菌8株(4.82%)和表皮葡萄球菌7株(4.22%)等。敏感药物(主要依据细菌检出率的前4种菌株的药敏试验)依次为:头孢菌素类、喹诺酮类、大环内酯类等。结论]定期、系统监测当地慢性化脓性中耳乳突炎的细菌耐药动态和流行动态,对临床合理使用抗菌素有重要的指导意义。

Microbiology and drug sensitivity analysis of chronic suppurative otitis media

Objective] To understand bacteriology of chronic suppurative otitis media (CSOM) and drug sensi- tivity of isolated bacteria in the last two years from Dalian for better treatment of CSOM. Methods] Secretions of 146 samples with chronic suppurative otitis media were collected and cultured for bacteria and fungi respec- tively. Drug sensitivity test of isolated bacteria was done. Result] Pathogens were found in 142 of 146 sam- ples, the main pathogens were aureus staphylococcus(34.34%), pseudomonas aeruginosa(21.08%) and bacillus proteus(9. 03%), and cephalosporin was showed the best susceptibility on those bacteria. Conclusion] To study microbiology and drug sensitivity of patterns with chronic suppurative otitis media was helpful to selecting an appropriate antimicrobial agent.
