摘    要:目的了解2006~2007年华东地区临床分离菌的分布和耐药特征,指导临床合理使用抗菌药物。方法采用纸片扩散法(KB法)或自动化仪器对2006~2007年华东地区25家医院临床分离菌株进行药敏试验及ESBLs的检测,按CLSI 2007年标准判定药敏结果,并用WHONET5.4软件进行数据分析。结果25家医院共分离菌种30215株,其中革兰阴性杆菌20579株,占68.1%,革兰阳性球菌9636株,占31.9%。大肠埃希菌和金葡菌分别占据革兰阴性杆菌和革兰阳性球菌的首位。大肠埃希菌、肺炎克雷伯菌和奇异变形菌中ESBLs的检出率分别为53.1%、36.3%和11.8%。葡萄球菌中MRSA和MRSCN的检出率分别为63.4%和81.9%;肠杆菌科细菌对碳青霉烯类抗生素高度敏感,耐药率<2%,但对其它抗菌药物的敏感率低于80%(奇异变形菌除外),铜绿假单胞菌对各种抗菌药物的敏感率均低于73%,鲍曼不动杆菌对各种抗菌药物的敏感率均低于70%。结论华东地区细菌耐药性比较严重,在全国范围内按统一方案开展耐药监测对合理使用抗菌药物和控制细菌耐药性的增加十分重要。

Abstract:Objective To investigate the distribution characteristics and drug resistance profile of clinical isolates in east China.Methods The susceptibility testing of clinical isolates in 25 hospitals from east China was performed and the ESBLs producing strains were detected using KB method or automated system.The results were analyzed by WHONET5 according to CLSI 2007.Results Of 30,215 clinical isolates,68.1% was Gram negative bacill and 31.9% was Gram positive cocci.E.coli was the most frequently isolated pathogen in Gram negative bacilli.Staphylococcus aureus was the most common pathogen in Gram positive cocci.63.4% of Staphylococcus aureus was MRSA and 81.9% of coagulase negative Staphylococcus were methicillin-resistant.ESBLs were produced in 53.1% of E.coli,36.3% of K.pneumonia and 11.8% of P.mirabilis isolates respectively.Conclusions Antimicrobial resistance in clinical bacterial isolates was a very serious promblem in east China.A nationwide surveillance system on antimicrobial resistance by a unified protocol was urgently needed for the rational use of antimicrobial agents and the control of bacterial resistance.
