腔镜下肝段切除术中B超引导与超声刀的联合应用

目的探讨腔镜超声引导联合超声刀的应用行肝解剖性肝段切除的可行性、安全性与临床价值。方法收集2007年1月~2008年12月我院行腹腔镜肝切除患者20例,所有患者均在全气腹条件下完成腔镜超声引导联合超声刀应用下肝解剖性肝段切除手术。结果 20例患者手术时间平均188min,出血量平均300ml。术中未出现不能控制的并发症,术后平均住院8.6d。结论腔镜超声引导联合超声刀以肝段为本的解剖性肝切除术更加符合肿瘤根治原则,值得在肝脏外科推广应用。

Anatomical segmental hapatectomy with ultracision-uarmaic scalpel under the guidance of ultrasongraphy

Objective To assess the feasibility、safety and clinical value of laparoscopic anatomical segmental hepatectomy with ultracision-uarmaic scalpel under the guidance of ultrasongraphy.Methods We performed laparoscopic liver resection in 20 patients form January 2007 to October 2008.All the cases received laparoscopic liver anatomy under endoscopic ultrasound guidance in complete pneumopertoneum.Results The mean operating time was 188min、and the mean blood loss was 300ml.There were no uncontrollable complications.The mean time of hospital stay after operation was 8.6d.Conclusion Segment-based hepatectomy with ultracision-uarmaic scalpel under the guidance of ultrasonography conforms to the principle in tumor treatment and is worthy of being applied in liver surgery.
