DHS、PFNA及人工关节治疗老年股骨粗隆间骨折临床疗效分析

摘    要:目的:探讨DHS、股骨近端防旋髓内钉(PFNA)及人工关节置换不同植入物治疗老年人股骨粗隆间骨折的临床疗效。方法:回顾性分析2005年1月~2010年1月以不同方法治疗老年股骨粗隆间骨折资料完整患者126例,其中DHS 46例、PFNA 38例及人工关节置42例,比较各组手术时间、术中平均出血量、围手术期并发症及Harris髋关节功能评分等指标。结果:本组126例病人出院后均得到随访,随访时间为6~60个月,平均18个月。内固定组均获得骨性愈合。手术时间PFNA组较其他2组短,差异有统计学意义(P〈0.05)。术中出血量和围手术期并发症上,PFNA组较其他2组少,差异有统计学意义(P〈0.05)。改良Harris髋关节功能评分上,PFNA及人工关节组较DHS组为优,差异有统计学意义(P〈0.05)。结论:DHS及PFNA固定与人工关节置换治疗老年股骨粗隆间骨折,均可获得满意的临床疗效。相对于DHS和人工关节置换,PFNA治疗股骨粗隆间骨折具有操作简单,出血少,手术时间短,固定可靠,尤其适用于老年患者的股骨粗隆间骨折。
