雌激素受体基因多态性与冠心病的关系

摘    要:目的 研究雌激素受体 (ER)基因多态性是否与冠心病 (CHD)有相关性。方法 用聚合酶链反应 限制性酶切长度多态性分析方法 ,对 72例CHD组患者和 5 3例对照组的ER基因多态性进行分析 ,比较ER基因多态性在两组之间和在CHD组不同临床表现 (稳定性心绞痛和急性冠脉综合征 )之间分布的差异性 ,并比较 3种ER基因型与冠心病相关的临床指标。结果 PP基因型在CHD组和急性冠脉综合征组中的分布频率明显高于对照组和稳定性心绞痛组 (P <0 .0 5 )。PP基因型的高血压病患病率 6 4 .0 % ,纤维蛋白原浓度 (3.6± 0 .8)g/L ,体重指数 2 5 .4± 2 .8和高密度脂蛋白胆固醇浓度 (1.0± 0 .2 )mmol/L ,与其他基因型有显著性差异 (P <0 .0 5 )。结论 ER基因多态性可能通过改变ER的表达 ,而影响ER介导的心血管保护作用。

Abstract:Objective To investigate the relationship between estrogen receptor gene polymorphism and coronary heart disease(CHD).Methods By polymerase chain reaction restriction fragment length polymorphism(PCR RFLP),ER gene polymorphism was analyzed in 72 CHD patients and 53 control individuals,and the distribution of ER genotype was compared in CHD group and control group,and in different CHD clinical situations.Based upon these,the clinical indexes associated with CHD of the three ER genotypes were compared.Results The proportion of PP genotype was higher in CHD group and acute coronary syndrome (ACS) group(27.8% and 30.9%)than in control group and stable angina pectoris (SAP) group(13.2% and 17.6%,P<0.05).There were significant differences between prevalence rate of hypertension(64.0%),fibrinogen concentration[(3.6±0.8) g/L],body mass index(BMI)(25.4±2.8),HDL C concentration[(1.0±0.2) mmol/L] in patients with PP genotype and those in patients with other genotypes(P<0.05).Conclusion ER gene polymorphism may affect cardiovascular protective effect mediated by ER through changing the expression of ER.
