LLF方案和PLF方案治疗晚期食管癌的疗效比较

目的比较由洛铂联合醛氢叶酸及氟尿嘧啶组成的LLF方案与顺铂联合醛氢叶酸及氟尿嘧啶组成的PLF方案治疗晚期食管癌的近期疗效及不良反应。方法将50例晚期食管癌患者随机分为LLF组和PLF组,各25例;分别采用LLF方案和PLF方案进行治疗,至少接受2个周期化疗,比较其近期疗效及不良反应,并定期随访。结果 LLF组和PLF组治疗后有效率分别为40%(10/25)和36%(9/25);疾病控制率分别为72%(18/25)和68%(17/25),两组患者近期客观疗效间差异无统计学意义(P>0.05)。两组患者均出现骨髓抑制和消化道反应,LLF组的恶心、呕吐等消化道反应发生率(28.0%)较PLF组(56.0%)显著降低,差异有统计学意义(P<0.05)。结论 LLF方案和PLF方案治疗晚期食管癌疗效和安全性均较好,不良反应能耐受,LLF方案较PLF方案消化道不良反应稍轻微。

Comparison of Efficacy of LLF Regimen and PLF Regimen for Advanced Esophageal Carcinoma

Objective To compare lobaplatin combined with 5-flu-orouracil and leucovorin and PLF regimen consisting of cisplatin combined with 5-FU and CF in the chemotherapy of advanced esophageal carcinoma.Methods Fifty patients with advanced esophageal carcinoma were treated by LLF regimen and PLF regimen,respectively.The response rate(RR)was evaluated after at least two cycles of chemotherapy when the toxicity was evaluated.Results There was no statistically significant difference between two groups in therapeutic effects.Both groups had bone marrow suppression and gastrointestinal tract reaction.Gastrointestinal tract reactions(nausea and vomiting)in LLF group were significantly less frequent than those in PLF group.Conclusion Both regimens were effective and well tolerated in patients with advanced esophageal carcinoma,and the gastrointestinal reaction in LLF group was less than that in PLF group.
