高压氧治疗脑梗死患者脑循环血流状态的分析

目的　探讨高压氧 (HBO)对急性脑梗死患者脑循环血流状态的影响。方法　选择 40例经确诊的急性脑梗死患者 ,应用高压纯氧及混合氧方案治疗 ,并应用无创性脑循环分析仪于治疗前、第一疗程结束时及第二疗程结束时监测脑循环血流状态 ,观察指标包括最大血流速度 (Vs)、最小血流速度 (Vd)、最大血流量 (Qs)及最小血流量 (Qd)。结果　治疗前 ,脑血管功能 (CVD)检测示脑梗死患者的双侧均有不同程度的降低 ,其中健侧与病侧的Vd和Qd存在着明显的差异 (均P <0 .0 1)。第一疗程结束时 ,Vs ,Vd ,Qs ,Qd与治疗前比较 ,均有明显的升高 (均P <0 .0 1) ,75 %患者恢复正常。而第二疗程后 ,Vs ,Vd ,Qs ,Qd亦明显高于治疗前及第一疗程结束时 ,95 %患者循环血流参数正常。结论　HBO可明显改善脑梗死患者的脑血流状态 ;而且血流状态的改变与临床症状的恢复成正比

Analysis of brain circular blood stream state of cerebral infarction treated with hyperbaric pure and mixtural oxygen

Objective To study the effect of hyperbaric oxygen(HBO) on brain circular blood stream state of acute cerebral infarction.Methods Hyperbaric pure oxygen and mixtural oxygen program were performed on 40 cases of final diagnotic patients with acute cerebral infarction.Brain circular blood stream state(including maximum velocity-Vs,minimus velocity-Vd,maximum quantity-Qs and minimus quantity-Qd)were monitored with non-wound brain circular analysis meter before treatment and finish of the first and the second course.Results Before treatment,the results of cerebral vessels determination (CVD) showed that the two sides of the patients decreased in different degree,and there were significant differences between Vd and Qd of sound side and that of sick side( P <0.01).And when the first course finished,Vs Vd,Qs and Qd all increased comparing with that before treatment( P <0.01),75% patients recovered normal.While the second course finished,Vs,Vd,Qs and Qd also significantly increased comparing with that before treatment and when the first course finished.95% patients were normal.Conclusion HBO can improve the brain circular blood stream state of patients with cerebral infarction;and the changes of blood stream state are in direct ratio to the recovery of clinical symptom.
