超声引导介入诊治细菌性肝脓肿57例的经验分析

目的总结超声引导介入诊治细菌性肝脓肿的部分值得注意的诊疗经验。方法本文患者均来自我科2003年10月至2009年12月诊治病例,对57例细菌性肝脓肿在超声定位引导下经皮肝穿刺,并对诊治经验进行分析。结果 57例病人31例放置引流管引流,抽出脓液20-350ml,行细菌培养,阳性39例（63.3%）,阴性18例（36.7%）。经药物冲洗治疗后,37例体温明显下降,超声随访1—3个月,脓腔消失52例,占94.1%。结论超声引导经皮肝穿刺引流治疗细菌性肝脓肿操作简单,提高了部分不典型病例的诊断率,避免了传统开腹手术带来的并发症,可以适应多种类型的细菌性肝脓肿的诊疗。

Ultrasound-guided interventional diagnosis and treatment of bacterial liver abscess with experience of 57 cases

Objective：To explore the value of ultrasound-guided interventional diagnosis and treatment of bacterial liver abscess.Methods：Patients in our department from October 2003 to December 2009 suspected bacterial liver abscess were enrolled.Ultrasound-guided percutaneous liver biopsy and treatment were done.Results：Thirty-one patients were placed drainage tube,and pus was 20 ～ 350 ml.Thirty-nine patients was positive（63.3%） and negative in 18（36.7%） with bacterial culture.After washout treatment,body temperature was decreased in 37 cases.With ultrasound follow-up for 1-3 months,the abscess disappeared in 52 cases,accounting for 94.1%.Conclusion：It is simple to treat bacterial liver abscess with ultrasound guided percutaneous puncture and drainage,and it can improve some of the detection rate of atypical cases.
