凯纷复合舒芬太尼用于乳腺癌术后静脉镇痛的临床观察

目的:观察凯纷复合舒芬太尼用于乳腺癌术后静脉镇痛(PCIA)的临床效果。方法:选择乳腺癌手术患者50例,ASAⅠ-Ⅱ级,随机分为2组。Ⅰ组观察组,给予凯纷100mg+舒芬太尼100ug+托烷司琼8mg,生理盐水稀释至50ml。Ⅱ组对照组,单纯用舒芬太尼150ug+托烷司琼8mg,生理盐水稀释至50ml。2组均于术毕即刻静脉连接自控镇痛泵。背景剂量0.5ml/h,冲击量0.5ml,锁定时间8分钟,记录术后2、4、8、12、24、48小时的视觉模拟评分(VAS)、Ramesay镇静评分及不良反应发生率。结果:2组病人镇痛效果良好,Ⅰ组患者术后2小时和4小时的Ramesay镇静评分低于Ⅱ组(P<0.05),其他时间段差异无统计学意义(P>0.05),恶心、呕吐、头晕、皮肤瘙痒等不良反应Ⅰ组明显少于Ⅱ组,有显著差异(P<0.05)。结论:凯纷复合舒芬太尼镇痛效果优于单纯舒芬太尼,且可降低不良反应的发生率。

The Clinical Observation of the Postoperative Analgesia with Flurbiprofen Axetil Combined with Sufentanyl in Patients Underwent Breast Carcinoma

Objective:To observe the analgesia effect of flurbiprofen axetil combined with sufentanyl in patients underwent breast carcinoma.Methods:Fifty patients underwent breast carcinoma were randomly divided into two groups equally with 25 cases each.Group I was administrated flurbiprofen axetil 100mg and sufentanyl 100ug and Tropisetron 8mg diluted to 50ml administrated via PCIA after surgery.Group Ⅱ was given sufentanyl 150ug and Tropi-setron 8mg diluted to 50ml via PCIA after surgery.The PCIA rate was 0.5ml/h,impulse 0.5ml,lock time 8min.Postoperative analgesia efficacy was assessed by visual analog scales(VAS)at 2h,4h,8h,12h,24h and 48h after surgery.The overall satisfaction to analgesic therapy was evaluated.Results:All the patients showed good pain relief with PCIA.The VAS of group Ⅱ at 2 h after operation was slightly lower than that of groupⅠ(P<0.05),which became similar 4 h later(P>0.05).The side effects were lower in group Ⅰ than those in group Ⅱ.It was significant different between two groups(P<0.05).Conclusions:Flurbipro-fen axetil combined with sufentanyl can obtain better analgesia effects to sufentanyl alone and can reduce side effects.
