不同切口部位在阑尾炎微创手术患者中的疗效比较研究

目的：比较阑尾炎微创手术中采用不同部位切口的疗效。方法：我院2012年3月至2013年3月诊断为阑尾炎并行微创手术患者共176例，按照住院号奇偶采取经脐切口或经脐周切口行微创手术，根据手术部位的不同分为经脐组和经脐周组，比较两组患者手术时间，引流管放置比例及出血量，切口感染、肠粘连发生率，疼痛指数，患者对手术切口外观及愈合满意度。结果：两组患者的手术时间，引流管放置比例及出血量，切口感染、肠粘连发生率、疼痛指数比较差异无统计学意义（P＞0．05）。对切口外观及愈合满意度比较，差异有明显的统计学意义（ P＜0．05）。结论：经脐切口与经脐周切口在阑尾炎微创手术患者的安全性和实用性无差别，但经脐切口术后愈合外观更符合患者对美容的要求。

Comparison of the Ther apy Effects of Different Incisions in Laparoscopic Appendectomy

Objective：To compare the effects of different incisions in laparoscopic appendectomy .Method：A retrospec-tive analysis was conducted for 176 patients diagnosed as appendicitis and received laparoscopic surgery in our hospital from March 2012 to March 2013.The patients were divided into intraumbilical group and periumbilical group according to the odevity of their admission number.Intraumbilical group was treated with the intraumbilical incision , and periumbilical group was treated with the periumbilical incision , followed by the traditional laparoscopic appendectomy .The operation time , usage percentage of drainage tube , amount of bleeding , incision infection rate , intestinal adhesion occurrence , pain index and patients＇satisfaction at the incision and wound recovery were compared .Res ult： There was no statistically significant difference in operation time , usage percentage of drainage tube , amount of bleeding , incision infection rate , intestinal adhesion occurrence and pain index （ P〉0.05 ） .The difference was statistically significant between two groups of patients on incision and wound recovery satisfaction score （P〈0.05）.Conclusion：The＇s no significant difference in the safety and feasibility .however, The intraumbilical incision seems to be more aritistic .
