全髋关节置换术与人工股骨头置换术治疗老年移位性股骨颈骨折的效果观察

目的：观察并比较全髋关节置换术（THA）与人工股骨头置换术（HA）治疗老年移位性股骨颈骨折的综合效果。方法：选取2010年7月至2012年10月本院收治的84例老年移位性股骨颈骨折患者为研究对象,将84例患者根据随机分配的方法分为A组（THA组）42例和B组（HA组）42例,然后将两组患者中不同Garden分型者的d＇Aubigne-postel评分、术后不同时间的疼痛评分及手术前后的生活质量评分进行比较。结果：A组患者中不同Garden分型者的d＇Aubigne-postel评分均好于B组,术后不同时间的疼痛评分均低于B组,生活质量评分高于B组,P均〈0.05,两组术后评估指标均有显著性差异。结论：THA术在老年移位性股骨颈骨折患者中的综合效果好于HA术,且对患者术后生存质量的改善作用也更为明显。

The Effect of Total Hiparthroplasty and Hemiarthroplasty in the Treatment of Elderly Patients with Displaced Femoral Neck Fractures

Objective： To observe and compare the comprehensive effect of total hiparthroplasty（ THA）and hemiarthroplasty（ HA） in the treatment of elderly patients with displaced femoral neck fractures.Method： 84 elderly patients with displaced femoral neck fractures in our hospital from July 2010 to October 2012were selected as research object,84 cases were divided into group A（ THA group 42 cases）,and group B（ HA group 42 cases）,by random method,then the d＇Aubigne-postel score of different Garden types,postoperative pain score at different times and preoperative and postoperative life quality score of two groups were compared.Result： The d＇Aubigne-postel score of group A with different Garden types were better than those of group B,postoperative pain score at different times were all lower than those of group B,life quality score were higher than those of group B,all P〈0.05,the postoperative evaluation indexes of two groups all had significant differences. Conclusion： The comprehensive effect of THA in the treatment of elderly patients with displaced femoral neck fractures are better than those of HA,and the improvement role for the postoperative life quality is more obvious.
