股骨近端髓内钉和近端解剖锁定钢板治疗股骨近端骨折合并股骨干骨折的手术方法和疗效探讨

比较股骨近端髓内钉法、近端解剖锁定钢板法分别治疗股骨近端骨折合并股骨干骨折的临床效果。方法：选取2010年1月至2013年6月期间46例在我院治疗的股骨近端骨折合并股骨干骨折患者，并随机分为观察组和对照组，每组23例；观察组采用股骨近端髓内钉法治疗，对照组采用近端解剖锁定钢板法；比较两组患者术后临床效果、不良情绪程度等。结果：观察组、对照组治疗总有效率分别为95．7％、73．9％，差异有显著性；手术、骨折愈合时间、术中失血量观察组均明显短（低）于对照组；术后不良情绪程度观察组患者显著轻于对照组，差异均具有统计学意义（ P＜0．05）。结论：股骨近端髓内钉术治疗股骨近端骨折合并股骨干骨折临床效果较佳，值得临床推广应用。

The Operation and Curative Effect of Proximal Femoral Intramedullary Nail and Proximal Anatomic Locking Plate in Treatment of Proximal Femoral Fractures Combined Femoral Fractures

To compare the clinical effect of proximal femur intramedullary nailing , proximal anatomic locking plate respectively treatment of proximal femoral fractures combined femoral fractures .Meth-od：46 cases of proximal femoral fractures from January 2010 to June 2013 in our hospital , and randomly were divided into observation group and control group , 23 cases in each group;the observation group with proximal femoral intramedullary nailing treatment , the control group was used proximal anatomic locking plate;And then compared two groups of patients with postoperative clinical effect and the degree of bad feel -ings etc.Result：Total effective rate in the observation group and the control group were 95.7%and 73.9%, it had significant difference;Surgery , fracture healing time and intraoperative blood loss of the observation group were significantly less than the control group; the degree of postoperative bad feelings in the observation group of patients were significantly lighter than the control group , the difference had statistically significant （P 〈0.05）.Conclusion： Proximal femoral intramedullary nailing treatment of proximal femoral fractures has better clinical effect of combined femoral fractures ,it is worthy of clinical popularization and application .
