口服米诺环素联合外用Derma光治疗重度寻常型痤疮临床观察

目的：探讨口服米诺环素联合外用Derma光治疗重度寻常型痤疮的临床疗效,为临床治疗提供理论依据。方法：选择符合标准的患者80例,采用随机数字的方法分为观察组和对照组各40例,对照组应用口服米诺环素,观察组应用口服米诺环素联合外用Derma光治疗,治疗8周后比较临床疗效。结果：治疗8周后观察组患者总有效率为97.50%,明显高于对照组的80.00%,差异有统计学意义（P〈0.05）。治疗前两组患者GAGS评分相似,差异无统计学意义（P〉0.05）;治疗后,两组患者GAGS评分较治疗前均下降,差异有统计学意义（P〈0.05）,但是观察组患者下降幅度更大（P〈0.05）。治疗期间观察组2例患者经红蓝光照射后面部皮肤出现轻度潮红、瘙痒和脱屑,对症处理1周后症状缓解。对照组未见不良反应。患者均随访3个月,观察组患者复发2例（5.00%）,对照组复发8例（20.00%）,差异有统计学意义（P〈0.05）。结论：口服米诺环素联合外用Derma光治疗重度寻常型痤疮的临床疗效确切,安全可靠,复发率低,值得临床推广使用。

Clinical Observation on Orally Taken Minocycline Combined with Externally Used Derma Light in Treatment of Severe Acne Vulgaris

Objective： To investigate the clinical therapeutic effects of orally taken minocycline combined with externally used Derma light on severe acne vulgaris,and to provide theoretic foundations for clinical treatment. Method： 80 patients meeting the criteria were selected and randomly divided into observation group and control group,40 patients in each group. The control group orally took minocycline,while the observation group orally took minocycline in combination with externally used Derma light,and the clinical therapeutic effects were compared 8 weeks after the therapies.Result： 8 weeks after the therapies,the total effective rate of the observation group was 97.50%,significantly higher than that of the control group（ 80.00%）,and the difference was statistically significant（ P＆lt;0.05）. Before the treatment,the GAGS scores in the patients of the two groups were similar,and the difference was statistically insignificant（ P＆gt;0.05）; after the treatments,the GAGS scores in the patients of both groups were decreased when compared with those before the treatments,and the differences were statistically significant（ P＆lt;0.05）,but the GAGS scores in patients of the observation group were decreased more significantly（ P＆lt;0.05）. During the treatment,2 patients of the observation group suffered from mild flushing,itching and desquamation in the facial skin after redblue light-struck,and the symptoms were relieved in 1 week after symptomatic treatments. No adverse reaction occurred in the control group. All the patients were followed up for 3 months,2 patients of the observation group relapsed（ 5.00%）,while 8 patients of the control group relapsed（ 20.00%）,and the difference was statistically significant（ P＆lt;0.05）. Conclusion： Orally taken minocycline combined with externally used Derma light in treatment of severe acne vulgaris has positive clinical therapeutic effects,It is safe and reliable,has low recurrence rate,and consequently is worthy of clinical application.
