腹腔镜胃袖套状切除术与 Roux-en-Y 胃肠短路术治疗肥胖疗效比较

目的：评价腹腔镜胃袖套状切除术（ laparoscopic sleeve gastrectomy ，LSG）与腹腔镜Roux-en-Y胃肠短路术（laparoscopic Roux-en-Y gastric bypass，LRYGB）治疗病态性肥胖的疗效差异。方法：选取我院2010年2月至2103年2月32例肥胖患者实行腹腔镜减肥手术，其中16例施行腹腔镜胃袖套状切除术（LSG），16例施行腹腔镜Roux-en-Y胃肠短路术（LRYGB），对比评估分析两组手术的手术时间、术中失血量、术后平均住院时间、术后排气时间、术后并发症以及术后3、6、12个月的体重指数减少值、多余体质量减少百分比。结果：所有患者均顺利完成手术，无1例中转开腹。 LSG组的手术时间、术中失血量、术后平均住院时间、术后排气时间均少于LYRGB，且有统计学意义（ P＜0．05）；而在术后3、6、12月体重指数减少值、多余体质量减少百分比方面则无明显差异（ P＞0．05）。结论：腹腔镜胃袖套状切除术达到与Roux-en-Y胃肠短路术同样的近期减重效果，并具有操作简单、术后恢复快、并发症少等优点，值得临床推广。

Comparing the Effect of Laparoscopic Sleeve Gastrectomy with Roux-en-Y Gastric Bypass Treatment for Morbid Obesity Patients

Objective： To initial contrast the effect of laparoscopic sleeve gastrectomy and laparoscopic Roux-en-Y gastric bypass treatment for morbid obesity patients. Method： From Feb. 2010 to Feb. 2013 ,a total of 32 cases morbid obesity patients in our hospital were recruited .Among 32 cases, 16 were treated by LSG and 16 were treated by LRYGB. Duration of surgery, intraoperative blood loss, days of hospitalization, the time needed for recovery of gastrointestinal function, the postoperative complications and 3, 6, 12 monthsof body mass index reduction value and excess weight loss percentage （%） were prospectively analyzed and evaluated. Result： All patients were successfully completed surgery, and none of the patient was converted to open surgery. Duration of surgery, intraoperative blood loss, days of hospitalization, the time needed for recovery of gastrointestinal function in the LSG group were significantly shorter than those in the LRYGB group （ P 〈0.05 ）. But there＇s no significant difference in 3, 6, 12 months of body mass index reduction value and excess weight loss percentage （P〉0.05）. Conclusion： LSG and LRYGB have some weight to lose as well, The benefits of LSG is easy operation, with quick rehabilitation, hypo-complication and so on.
