短暂性脑缺血发作进展为脑梗死的早期危险分层和干预的临床研究

目的：对短暂性脑缺血发作（TIA）病例进行风险性分层,并对特定危险组给予积极临床干预,评价其在降低脑梗死发生率中的价值。方法：160例TIA住院患者,随机分为研究组和对照组各80例。研究组使用改良的ABCD2评分方法进行低危、中危、高危分组,中危以上病例在常规治疗基础上,加用低分子肝素钙和奥扎格雷钠两种药物治疗,记录发病7d内的脑梗死发生率;对照组不进行危险分层,使用常规治疗,记录7d内脑梗死发生率。结果：研究组脑梗死发生率为5%（4例）,对照组脑梗死发生率为16.25%（13例）。两组脑梗死发生率比较,研究组低于对照组,有显著性差异（p〈0.05）,治疗过程无严重的或导致治疗中止的不良反应。结论：本方法对降低TIA进展为脑梗死发生率、改善TIA预后具有临床价值。

The Research of Early risk Stratification and Intervention of TIA Progressing into Cerebral Infarction

Objective： Transient ischemic attack patients were divided into low,medium and high risk group,and the specific risk group gave a positive clinical intervention.To evaluate its value to reduce the incidence of cerebral infarction. Method： 160 patients were from clinic or hospital at chaozhou hospital. They were randomly divided into two groups： research group and control group（ n = 80） 。The research group redivided into low,medium and high risk group by improve ABCD2 grade,medium and high risk group add FraxiParine and ozagrel sodium on the basis of normal treatment,the control group were given normal treatment,recorded incidence rates of cerebral infarction in the two groups. Result： The incidence rate of the research group was 5%,and the rate of control group was 16.25%. Two groups incidence rate comparison,the research group was lower than that of control group. There is significantly statistical difference（ P ＆lt; 0. 05）.Conculsion： The technique has great value of preventing transient ischemic attack progressing into Cerebral infarction.
