奥利沙铂为基础的方案治疗老年晚期结直肠癌的疗效和安全性探讨

目的：探讨奥利沙铂为基础的方案治疗老年晚期结直肠癌的临床疗效和安全性。方法：将66例晚期结直肠癌患者采用数字表法随机分为两组，每组各33例，其中对照组采取卡培他滨＋奥利沙铂化疗方案，观察组采取雷替曲塞＋奥利沙伯化疗方案，比较两组近期疗效及不良反应发生情况。结果：观察组有效率和疾病控制率分别为39．39％和78．79％，均显著高于对照组的21．21％和60．60％（ X2＝6．587，．13，P＜0．05）；两组主要不良反应有恶心、呕吐、疲劳、中性粒细胞减少、血小板减少及周围神经毒性等，且以Ⅰ、Ⅱ度最为常见，其中观察组恶心、呕吐发生率为51．51％，显著低于对照组的69．70％（ X2＝7．35，P＜0．05），其余不良反应发生率比较均无显著性差异（ P＞0．05）。结论：雷替曲塞联合奥利沙伯方案治疗老年晚期结直肠癌疗效显著，安全性高，具有较好的临床应用价值。

Effect and Safety of Oxaliplatin-based Regimens in Treatment of Elderly Patients with Advanced Colorectal Cancer

Objective：To investigate the effect and safety of oxaliplatin-based regimens in treatment of elderly patients with advanced colorectal cancer .Mtehod： 66 cases of patients with advanced colorectal cancer were randomly divided into two groups according to digital table method , each group with 33cases. The control group took capecitabine and oxali platinum chemotherapy , while the observation group took raltit-rexed and oxaliplatin chemotherapy , the efficacy and adverse reactions were compared .Rseult：The effective rate and disease control rate in observation group was 39.39%and 78.79%respectively , which was signifi-cantly higher than the control group （21.21%and 60.60%） （ X2=6.58, 7.13,P〈0.05）;main adverse reac-tions in the two groups were nausea , vomiting, fatigue, neutropenia, thrombocytopenia and peripheral nerve toxicity, and the Ⅰ,Ⅱdegree was the most common , the occurrence rate in of nausea , vomiting observa-tion group was 51.51%, which was significantly lower than the control group （ 697.0%） （ X 2=7.35, P〈0. 05）, the incidence adverse reactions of two groups showed no significant difference （P〉0.05）.Conclusio n：Raltitrexed combined with oxaliplatin regimens has a significant effect , with higher safety and good clinical value in treatment of elderly patients with advanced colorectal cancer .
