ABPM及hs-CRP对高血压患者尿微量白蛋白的预测意义

目的：探讨高血压动态血压监测（ABPM）联合高敏C-反应蛋白（hs-CRP）对尿微量白蛋白预测价值。方法：对所有入选的高血压患者询问病史、体检并行动态血压监测（ABPM）。检测hs-CRP、尿微量白蛋白及血生化。根据检测结果将高血压患者分为微量蛋白尿组（实验组）和无微量蛋白尿组（对照组）。结果：高血压合并微量蛋白尿患者昼夜节律消失率、24h SBP、dSBP、nSBP、24h PP、dPP、nPP、hs-CRP明显高于单纯高血压组,差异有统计学意义（P〈0.01）;而24h DBP、dDBP、nDBP差异无统计学意义（P〉0.05）。结论：血压节律消失、收缩压、脉压及hs-CRP是高血压患者微量蛋白尿重要的预测因子,ABPM以及hs-CRP对高血压患者肾脏早期损害有较好的预测价值。

The Predictive Value of the Ambulatory Blood Pressue Monitoring and hs-CRP to the Microalbuminuria in Patients with Essential Hypertension

Objective： To study the predictive value of the ambulatory blood pressue monitoring（ ABPM） and high sensitivity C-reactive protein（ hs-CRP） to the microalbuminuria in patients with essential hypertensives.Method： The patients were inquired disease history and physical examination and ambulatory blood pressure monitoring,carotid were carried out. hs-CRP,microalbuminuria and blood biochemistry were detected.The patients were divided into two groups： microalbuminuria group（ experimental group） and nonmicroalbuminuria group（ control group） according to the detected results.Result： The level of the 24h SBP,dSBP,nSBP,24h PP,dPP,nPP,the ratio of Non-dipper blood pressure and hs-CRP in experimental group were high than that in control group,there was statistical significance（ P〈0.01）. 24h DBP,dDBP,nDBP in two groups were no statistical significance（ P〈0.05）. Conclusoin： It＇s an important predictor of microalbuminuria that there＇s the disapeared of blood pressure rhythm,systolic blood pressure,pulse pressure and hsCRP.ABPM and hs-CRP has better predictive value on early renal damage in patients with essential hypertension.
