腰大池穿刺持续引流联合尼莫地平治疗外伤性蛛网膜下腔出血临床疗效观察

目的：探讨腰大池穿刺持续引流联合尼莫地平治疗外伤性蛛网膜下腔出血的治疗效果。方法：外伤性蛛网膜下腔出血178例随机分为两组，传统基本治疗组89例应用传统基本脱水降颅压、保护脑细胞、营养脑神经等综合基本治疗；联合治疗组89例在传统基本治疗的基础上入院立即给予尼莫地平注射液持续静脉泵点，于发病后24h给予腰大池穿刺持续引流。结果：联合治疗组治疗后脑血管痉挛的发生率、并发症发生率、患者住院天数明显低于对照组，差异有统计学意义（ P＜0．05）。结论：腰大池穿刺持续引流联合尼莫地平治疗外伤性蛛网膜下腔出血，在降低脑血管痉挛的发生率、并发症（如脑梗塞、再出血、脑积水等）发生率，恢复脑功能，减少患者住院天数方面，治疗效果确切，患者预后改善明显。此方法值得临床广泛推广应用。

Continuous Lumbar Drainage Combined with Nimodipine Injection in the Treatment of Traumatic Subarachnoid Hemorrhage

Objective：To investigate the treatment effect of continuous lumbar drainage combined with nimodipine injection in the treatment of traumatic subarachnoid hemorrhage .Method：178 patients with trau-matic subarachnoid hemorrhage in total were randomly divided into treatment group and control group , 89 pa-tients in each group .Both groups were given traditional routine medical treatments like dehydration , reduce intracranial pressure , neuroprotective .Continuous intravenous nimodipine injections were given to treatment group as soon as the patients were sent to hospital , and continuous lumbar drainage were given 24 hours lat-er.Result： The patients in treatment group had lower incidence of cerebral vasospasm and complications such as cerebral infarction , rebleeding , hydrocephalus as well as less hospitalization days compared with pa-tients in control group , the difference was significant （ P〈0.05） .Conclusion： Continuous lumbar drainage combined with nimodipine injection has a good curative effect on traumatic subarachnoid hemorrhage , and can significantly reduce the incidence of cerebral vasospasm and complications as well as hospitalization days . The treatment is worthy of promotion for the better prognosis of patients .
