皮肤软组织扩张器及重组人表皮生长因子在面颈部瘢痕整形中的应用研究

目的：探讨重组人表皮生长因子（ rhEGF）在皮肤软组织扩张器治疗面颈部瘢痕中的临床效果及安全性。方法：选择符合标准的患者40例，随机分为观察组和对照组各20例，对照组应用皮肤软组织扩张器治疗，观察组加用rhEGF，比较二者临床效果。结果：观察组患者扩张至额定容量所需时间、皮肤扩张率及软组织及时回缩率均优于对照组，差异有统计学意义（P＜0．05）。观察组患者供皮区创面羟脯氨酸（ OHP ）含量高于对照组，而I/Ⅲ型胶原比值低于对照组，差异有统计学意义（ P＜0．05）。结论：面颈部瘢痕应用皮肤软组织扩张器扩张皮肤期间，加用rhEGF可促进皮肤扩张，缩短扩张时间，减少软组织即时回缩率，方法简单，值得临床推广应用。

The Application of Skin Soft tissue Expander and Recombinant Human Epidermal Growth Factor in the Plastic Surgery of Facial and Cervical Scars

Objective： To investigate the safety and clinical effects of recombinant human epidermal growth factor （ rhEGF） applied in the plastic surgery of facial and cervical scars with the help of a skin soft tissue expander .Method：40 eligible patients were selected and randomly divided into observation group and control group , 20 patients in each group;the control group used a skin soft tissue expander , while the obser-vation group additionally used rhEGF , and the clinical effects of the two groups were compared .Result：The required time for expanding to a rated capacity , skin expansion rate and soft tissue prompt-retraction rate in the patients of the observation group were all better than those of the control group , and the differences were statistically significant （P〈0.05）.The hydroxyproline （OHP） content in the wound surface at the donor site in the patients of the observation group was higher than that of the control group , while the I/Ⅲ-form colla-gen ratio in the patients of the observation group was lower than that of the control group , and the differences were statistically significant （P〈0.05）.Conclusion：In the period of skin expansion in a plastic surgery of fa-cial and cervical scars using a skin soft tissue expander , additional use of rhEGF can promote skin expan-sion , shorten skin expansion time and reduce soft tissue prompt-retraction rate , It is easy and feasible , and consequently it is worthy of clinical application .
