综合超声检查技术在肺栓塞诊断中的应用价值

目的　探讨超声心动图及下肢静脉高频超声联合检查在肺栓塞诊断中的临床应用价值。方法　利用超声心动图、下肢静脉高频超声对 31例临床疑诊的急、慢性肺栓塞患者进行了检查。结果　 31例疑诊患者经临床综合确诊 15例。在确诊的 15例肺栓塞患者中 ,超声心动图检查示右心系统扩大 14例、肺动脉增宽 13例、室间隔运动异常 9例、三尖瓣反流 14例、肺动脉收缩压增高 13例、肺动脉主干内血栓 1例 ;下肢静脉高频超声示下肢深静脉血栓形成 6例。结论　超声心动图及下肢静脉高频超声联合检查技术对肺栓塞的筛查及诊断具有重要的临床应用价值 ,对临床疑诊肺栓塞患者做超声心动图检查时宜常规同时行下肢深静脉超声检查。

Clinical value of comprehensive ultrasonic examination in heart and deep veins of lowerlimbs for the diagnosis of pulmonary embolism

Objective To assess the role of echocardiography and high-frequency ultrasonic examination in deep veins of lower limbs for the diagnosis of pulmonary embolism. Methods Thirty-one patients with suspected acute or chronic pulmonary embolism were investigated via echocardiography and high-frequency ultrasonic examination in deep veins of lower lambs. Results Fifteen of 31 patients with suspected pulmonary embolism were diagnosed as pulmonary embolism clinically. In all of the 15 patients, typical right heart enlargement was showed in 14 patients, pulmonary artery dilation and pulmonary hypertension were showed in 13 patients,motion abnormality in interventricular septum was found in 9 patients, tricuspid regurgitation was showed in 14 patients, and the embolus within main pulmonary artery was found in one patient by echocardiography. Thrombi within deep veins of lower limbs were found in 6 patients by high-frequency ultrasonography. Conclusion Combination of echocardiography and high-frequency ultrasonography in deep veins of lower limbs plays an important role for the diagnosis of pulmonary embolism. The ultrasonic examination in deep veins of lower limbs should be performed in patients with suspected pulmonary embolism.
