摘    要:目的通过研究子宫肌瘤动态增强磁共振成像(dynamic contrast-enhanced MRI,DCE-MRI)定量参数与治疗效果的相关性,探讨定量参数对子宫肌瘤高强度聚焦超声(high intensity focused ultrasound,HIFU)治疗效果即时评估的应用价值。材料与方法回顾分析子宫肌瘤HIFU治疗的13个病例,根据治疗后DCE-MRI表现分为完全消融组及不完全消融组。应用Mann-Whitney U秩和检验和Spearman秩相关分析研究定量参数与分组的关联。结果 Ktrans(治疗后)、Ve(治疗后)、Vp(治疗后)与分组呈正相关,相关系数分别为r=0.713、0.757和0.668;Ktrans(变化率)、Ve(变化率)与分组呈负相关,相关系数均为r=-0.802,且相关性更显著。结论 DCE-MRI定量参数对子宫肌瘤HIFU治疗疗效即时评估具有参考价值。

The preliminary value of DCE-MRI quantitative parameters in high intensity focused ultrasound (HIFU) ablation's immediate effect for uterine ifbroids
Abstract:AbstractObjective:To evaluate the immediate value of dynamic contrast-enhanced MRI(DCE-MRI)quantitative parameters after HIFU treatment of uterine fibroids. Materials and Methods:Thirteen patients with uterine fibroids were scanned with DCE-MRI before and after HIFU ablation. They were grouped into two groups, the complete ablation group and the non-complete ablation group according to appearance of DCE-MRI.Mann-Whitney U test andSpearman's correlations were applied to ifnd the association between quantitative parameters and grouping.Results:Ktrans(after treatment), Ve (after treatment) and Vp (after treatment) were positively correlated with the grouping, and correlation coefifcents were 0.713 , 0.757 and 0.668 respectively. Ktrans(rate of change)and Ve (rate of change) were negatively correlated with the grouping,and correlation coefifcients were both -0.802.Conclusion:DCE-MRI quantitative parameters have reference value in HIFU treatment's immediate effect evaluations for uterine ifbroids.
