纤维支气管镜管理现状调查

目的：调查纤维支气管镜在临床的分布状况、管理现状和运作模式。方法：采用便利抽样法,自行设计问卷对60所医院进行调查。结果：纤维支气管镜的分布涉及多学科领域,病例来源广泛;集中在消毒供应中心管理的医院有5所（8．33％）、内镜清洗中心的19所（31．67％）;清洗消毒设施欠完善,具有内镜清洗消毒机的医院有19所（31．67％）,仅由专职护士清洗的42所（70．00％）,多酶清洁液无一洗一换的21所（35．00％）,戊二醛浸泡≤20min的5所（8．33％）,仅目测进行清洗效果监测的16所（26．67％）。结论：强化管理理念,配备必要的清洗消毒设备,严格专业技能培训,规范操作流程管理,加强效果监测是纤维支气管镜清洗消毒质量保障的关键措施。而集中管理有助于提升医疗安全与品质,减少医院感染风险,有效节约卫生资源,充分发挥专业优势。

Survey on management and operation pattern of fiberoptic bronchoscopy

Objective： To survey the distribution, centralized management and operation pattern offiberoptic bronchoscopy. Methods： By convenience sampling method＇, questionnaire survey was taken among 60 hospitals. Results： Distribution of fiberoptic bronchoscopy was related to many disciplines and a wide source of cases. Centralized management was mainly taken by sterilization and supply centers （5 cases, 8.33%） and endoscope cleaning centers （19 cases, 31.67%）. Cleaning and sterilizing facilities needed to be improved. 19 hospitals had cleaning and sterilizing facilities for fiberoptic bronchoscopy （31.67%）; 42 hospitals had professional nurses to take charge of cleaning （70.00%）; 21 hospitals did not use disposable multi-enzyme cleansing solution （35.00%）, 5 hospitals used 2% alkaline glutaraldehyde but their soak period was less than 20 minutes （8.33%）, and 16 hospitals did monitoring only by visual inspection. Conclusion： In order to guarantee the quality of cleaning and sterilizing of fiberoptic bronchoscopy, the concept of management should be strengthened, essential cleaning and sterilizing equipments should be provided, professional skills training should be enhanced and operation procedure should be standardized. Centralized management would be an effective way to improve medical safety and reduce risk of infection. Moreover, resources can be effectively saved and professional advantages can be fully used.
