造血干细胞移植层流病房护理安全管理隐患及对策

目的：分析层流病房存在的主要护理安全隐患及相关因素，探讨造血干细胞移植患者的护理安全管理。方法：回顾分析我院血液内科层流室2008年1月至2010年12月行造血干细胞移植78例患者的护理资料，分别从管理体制、护理人员素质、层流环境和设施以及患者和家属的配合等方面进行不安全因素的分析，并根据存在的问题采取相应的对策实施安全管理，并统计分析实施安全管理前、后3年护理不良及隐患事件的发生率，比较两者的不同。结果：通过完善层流室的制度、流程，加强护理人员的业务知识、法律、法规学习，安全管理用药，合理配置人力，改善超负荷工作状态，减轻护士的身心压力等，护理不良事件及隐患事件的发生率明显降低，差异具有统计学意义。结论：针对影响层流病房安全隐患的因素采取相应的防范措施，可提高造血干细胞移植患者安全管理质量。

Nursing security hidden danger and countermeasures of laminar flow ward for hematopoietic stem cell transplantation

Objective：To analyze the main nursing security hidden danger and related factors existing in laminar lfow ward, and explore the nursing safety management for hematopoietic stem cell transplantation patients. Methods：Nursing data of 78 hematopoietic stem cell transplantation patients from January, 2008 to December, 2010 were retrospectively analyzed. The hidden dangers in several aspects, including the management system, the quality of nursing staff, laminar lfow environment, facilities, and cooperation of patients and families, were summarized and analyzed. And then safety management countermeasures were implemented accordingly. After 3 years of implementation, adverse and hidden danger nursing events before and after safety management were analyzed and compared. Results：The adverse and potential danger nursing events were statistically decreased after the safety management, which included improving the system and process of laminar lfow ward, strengthening knowledge, laws and regulations training of nurses, enhancing medication safety management, reasonable allocation of human resources to reduce the workload, physical and mental stress of nurses. Conclusion：Preventive measures according to the factors related to hidden danger should be implemented to improve the safety management quality of hematopoietic stem cell transplantation patients.
