胃镜胃管合并使用在抢救急性有机磷农药中毒中的临床效果观察

目的探讨胃镜胃管合并使用在胃镜直视下抢救急性有机磷农药中毒的临床作用。方法选取有机磷农药中毒患者83例，分实验组41例，对照组42例，比较两组洗胃时间、不良反应及并发症、插管一次性成功率、阿托品化出现时间、住院时间和抢救成功率。结果实验组：洗胃时间为（25．46±6．34）min、不良反应及并发症发生率为9．76％、插管一次性成功率为95．12％、阿托品化时间为（3．27土1．62）h、住院时间为（7．21±3．34）d、抢救成功率为97．56％；对照组：洗胃时间为（35．31±8．50）min、不良反应及并发症发生率为73．81％、插管一次性成功率为73．81％、阿托品化时间为（6．58±4．25）h、住院时间为（13．24±4．38）d、抢救成功率为78．57％，两组比较有显著差异性（P〈0．01）。结论胃镜胃管合并使用在胃镜直视下抢救有机磷农药中毒能明显地提高疗效。

Gastric Lavage Combined with Endoscopy in Management of Acute Organophosphorus Pesticide Poisoning:an Observation of Clinical Outcome

Objective To evaluate the clinical effect of gastric lavage combined with endoscopy in the management of acute organophosphorus pesticide poisoning（AOPP）. Methods Eighty-three AOPP patients were randomly divided into treatment group （41 cases） and control group （42 cases）. Patients in the treatment group were given gastric tube lavage combined with endoscopy and those in the control group were given conventional gastric lavage. The time of gastric lavage, adverse effects, complications, the successful rate of first intubation, atropinization, hospital stay, and survival rates were compared between the 2 groups. Results The time of gastric lavage, the incidence of adverse effect and complication,the successful rate of first intubation, the time of atropinization,the hospital stay, and survival rate in treatment group were （25. 46 ± 6. 34）min, 9.76%,95.12%, （3.27±1.62）h, （7.21±3.34）d, and 97.56%, respectively; and those in control group were （35.31± 8. 50）min,73.81%, 73. 81%, （6. 58±4.25）h, （13.24±4.38）d, and 78.57%, respectively; there were significant differences between the 2 groups（ P〈0. 01）. Conclusion Gastric lavage combined with endoscopy can obviously improve the clinical outcome of AOPP management.
