过敏性紫癜肾炎患儿血清hs-CRP,MMP-9的表达意义

目的探讨过敏性紫癜肾炎患儿血清超敏C反应蛋白（hs-CRP）,金属蛋白酶-9（MMP-9）的表达意义。方法 40例过敏性紫癜肾炎患儿作为观察组,40例无肾受累的过敏性紫癜患儿作为对照组,30例健康儿童作为健康组。测定入选人群的血清hs-CRP和MMP-9。结果观察组和对照组患者血清hs-CRP、MMP-9明显高于健康儿童（P〈0.01）,观察组明显高于对照组（P〈0.01）。观察组患儿24 h尿蛋白定量为（2.53±0.91）g,与患儿血清hs-CRP呈明显正相关关系（r=0.6440,P〈0.05）;与患儿血清MMP-9有明显正相关关系（r=0.2164,P〈0.05）。本次随访6月观察组患儿未出现肾衰竭,临床康复组和轻度尿异常组血清hs-CRP、MMP-9明显高于活动性肾脏病组（P〈0.01,P〈0.05）,并且临床康复组hs-CRP、MMP-9明显高于轻度尿异常组（P〈0.01）。结论血清hs-CRP和MMP-9可以作为过敏性紫癜肾炎患儿病情和预后监测的重要指标。

The value of hs-CRP and MMP-9 in serum in children with allergic purpura nephritis

Objective To study the value of high sensitivity C-reactive protein（hs-CRP） and matrix met alloproteinase-9（MMP-9） in serum in children with allergic purpura nephritis.Methods 40 children with allergic purpura nephritis were enrolled into the observation group,40 children with allergic purpura non-nephritis were enrolled into the control group and 30 healthy children were enrolled into the healthy group.The hs-CRP and MMP-9 in serum were detected.Results The hs-CRP and MMP-9 in the observation group and the control group were higher than these in the healthy group（P 0.01）.The hs-CRP and MMP-9 in the observation group were higher than these in the control group（P 0.01）.24-hour urinary protein in the observation group was（2.53 ±0.91） g,which had a positive correlation with hs-CRP（r =0.6440,P 0.05） and a positive correlation with MMP-9（r = 0.2164,P 0.05）.They were followed up for six months.The hs-CRP and MMP-9 in the clinical rehabilitation group and mild urinary abnormalities group were higher these in the activities kidney disease group（P 0.01,P 0.05）.The hs-CRP and MMP-9 in the clinical rehabilitation group were higher these in mild urinary abnormalities group（P 0.01）.Conclusion Serum hs-CRP and MMP-9 can be used as an important indicator of allergic purpura nephritis in children with the disease and prognostic monitoring.
