自制引流装置经阴道腹膜内外同时引流在宫颈癌术后的应用研究

目的研究自制引流装置经阴道腹膜内外同时引流在子宫广泛切除加盆腔淋巴结清扫术患者术后引流效果,以减少宫颈癌术后并发症的发生。方法搜集49例行自制引流装置经阴道腹膜内外同时引流者为研究组,33例经下腹壁两侧腹膜外引流者为对照组。观察两组术后引流量、引流时间、术后体温情况、淋巴囊肿发生率、肠蠕动恢复时间及术后首次下床时间。结果研究组术后盆腔引流量明显多于对照组（P〈0.05）,术后恢复正常体温时间、肠蠕动恢复时间、首次下床时间明显早于对照组（P〈0.05）,淋巴囊肿发生率明显少于对照组（P〈0.05）,术后引流时间两组比较差异无统计学意义（P〉0.05）。结论自制引流装置经阴道腹膜内外同时引流,引流效果明显优于经下腹壁两侧腹膜外引流,可有效减少宫颈癌术后并发症的发生。

Clinical Study of the Self-made Drainage Device Transvaginal inside and outside the Peritoneum in the Post-operative Patients with Cervical Cancer

Objective To study the clinical effect of the self-made drainage device transvaginal inside and outside the peritoneum in the extensive hysterectomy and pelvic lymph node resection of post-operative patients.Methods A total of 82 patients were operated with extensive hysterectomy and pelvic lymph node resection in our hospital from 2005.02 to 2010.02.49 cases which routine transvaginal self-made drainage device while drainage inside and outside the peritoneum as study group,33 cases following abdominal wall on either side of the peritoneum drainage as control group.Compared the volume of cited flow,draining time,the condition of temperature,the incidence rate of lymphatic cyst,and the first time of getting out of bed after operation between the two groups.Results The volume of cited flow in the study group was significantly more than that in the control group （P0.05）,The time of getting out of bed and returning to normal temperature in the study group were more earlier than those in the control group（P0.05）,The incidence rate of lymphatic cyst in the study group was significantly lower than that in the control group（P0.05）.Conclusion The effect of the self-made drainage device transvaginal inside and outside the peritoneum is significantly better than the lower abdominal wall on either side of the peritoneum drainage,It can effectively reduce the incidence of complications of the postoperative patients with cervical cancer.
