高龄孕产妇妊娠与分娩干预及对策探讨

目的了解高龄孕妇的妊娠结局，为提高该高危人群的围产期预后提供指导。方法回顾性分析2007年6月—2009年6月分娩的360例年龄35～46岁高龄孕妇的妊娠结局，并随机选择同期分娩的年龄介于22～34岁的孕妇312例作为对照。结果高龄孕产妇在妊娠戎、分娩期并发症和合并症发生率，均高于对照组（P〈0．05），围产儿结局中仅新生儿体重低于正常对照组。结论高龄妊娠分娩母儿风险明显上升。阴道自然分娩率下降，剖宫产明显升高，应加强对高龄产妇的产前检查及产时监护，以提高风险意识，确保母儿安全。

Pregnancy Outcome for Women with Advanced Maternal Age

Objective To evaluate pregnancy outcome in 360 women with advanced maternal age and to improve their prognosis during perinatal. Methods A retrospective study was undertaken for 360 women aged between 35 and 46 years old who delivered in Dong - fang Hospital of Pizhou City from 2007 to 2009, Meanwhile 312 women aged between 22 and 34 years old who delivered at the sametime served as control group. Results Of the elderly in maternal pregnancy, childbirth complications and the incidence of complications were higher （P 〈 0. 05 ） , except that the perinatal outcome birth weight lower than that of the control group. Conclusion The risk of pregnant women and infant increases obviously in older pregnancy, the rate of spontaneous vaginal delivery decreases, and the rate of caesarean section increases. In order to improve pregnancy outcome, it is important to strengthen the intrapartum guardianship and prenatal examination of older pregnancy.
