经内镜下注射人纤维蛋白粘合剂治疗7例食管癌术后胸内吻合口漏临床研究

目的：探讨经内镜下注射人纤维蛋白粘合剂在食管癌术后胸内吻合口漏患者治疗中的应用效果。方法回顾性分析7例术后发生胸内吻合口漏的胸内吻合的食管癌患者临床资料，比较内镜下注射人纤维蛋白粘合剂治疗前后患者吻合口旁引流量、体温、血C反应蛋白（C-reaction protein，CRP）、血白细胞计数等指标的变化。结果经内镜下注射人纤维蛋白粘合剂治疗后7患者吻合口旁平均引流量、白细胞计数、CRP水平均明显降低，与治疗前比较差异具有统计学意义（P＜0.05）。治疗前后患者体温差异无统计学意义（P＞0.05）。结论经内镜下注射人纤维蛋白粘合剂封闭吻合口漏口可有效降低漏口周围渗出，减轻患者全身炎症反应，促进患者术后恢复。

The application of endoscopic injection of fibrin sealant in repair of intrathoracic anastomotic leakage after radical resection of esophageal carcinoma

Objective To evaluate the clinical value of endoscopic injection of fibrin sealant in patients with intra-thoracic anastomotic leakage after radical resection of esophageal carcinoma. Methods The clinical data of 7 pa-tients suffering intrathoracic anastomotic leakage were analyzed. The drainage volume adjacent to the anastomosis, body temperature, white blood cell (WBC) count and CRP of those patients before and after the treatment were com-pared. Result The drainage volume adjacent to the anastomosis, WBC and CRP of the 7 patients were significantly decreased after endoscopic injection of fibrin sealant therapy, and significant differences were observed in comparison between the pre- and post-treatment values of these measures (P<0.05). While the body temperature was stable in all patients after treatment (P>0.05). Conclusion Endoscopic injection of fibrin sealant is effective in reducing anasto-motic exudates, alleviating systemic inflammatory response, and may promote postoperative recovery.
