经左胸廓单切口食管癌切除术的疗效及术后生活质量评价分析

目的：分析经左胸廓单切口食管癌切除术的临床疗效及患者术后的生活质量情况。方法将110例食管癌患者，随机分为观察组和对照组，每组55例患者，观察组采取经左胸廓单切口切除手术，对照组进行常规的手术，比较两组患者手术时间、淋巴结清扫数量、住院时间、失血量、并发症发生情况及术后生活质量评分。结果观察组患者手术时间和住院时间均较对照组短，术中失血量少于对照组，治疗后观察组患者生活质量量表（QoL）评分显著高于对照组，且乳糜胸、切口感染、肺部并发症发生率显著低于对照组，差异均有统计学意义（P<0.05）；两组术中淋巴结清扫数目，术后吻合口瘘、喉返神经损伤发生率比较差异均无统计学意义（P>0.05）。结论经左胸廓单切口进行食管癌切除可提高患者术后生活质量，减少并发症的发生，有利于患者早日康复。

The quality of life for the left thoracic esophagectomy in the treatment of esophageal cancer

Objective To explore the curative effect and complications of the left thoracic esophagectomy in the treatment of esophageal cancer. Method A total of 110 cases of esophageal cancer patients were randomized to ei-ther experimental group or control group, with 55 cases in each; The experimental group was given the left thoracic esophagectomy alone, while the control group was administered with conventional surgery; The operation time, num-ber of resected lymph nodes, hospital stay, blood loss, complications and postoperative quality of life (QoL) scores of both groups were compared. Result Patients in the experimental group had shorter operation time, less intra-opera-tive blood loss, and higher postoperative QoL score compared with the control group, while the incidences of chylo-thorax, incision infection, and pulmonary complications in experimental group were significantly lower than that of the control group (P<0.05); And the number of resected lymph nodes, incidences of postoperative anastomotic fistula, recurrent laryngeal nerve injury were similar in the two groups (P>0.05). Conclusion The left thoracic esophagecto-my may improve patient ’s quality of life, reduce the incidences of complications, thus it ’s able to promote postoper-ative recover.
