420例结核性胸膜炎和14例癌性胸膜炎对照观察

本文通过两组结核性胸膜炎420例,癌性14例的对照观察,发现结核性胸膜炎有如下特点:年龄多在40岁以下,急性起病,主要症状为发烧、乏力、胸痛、咳嗽、积液多数为少量,胸水多为渗出性,结核菌素试验强阳性,对抗痨药物治疗效果明显。癌性胸膜炎有如下特点:年龄多在50岁以上,临床表现为进行性胸痛和呼吸困难,多为中等量积液,右侧多发,常为渗出液,结素试验阴性,抗结核药物治疗无效。避免将癌性胸膜炎误诊为结核性胸膜炎和结核性胸膜炎皮质激素的使用原则是:强有力抗痨方案基础上加以适量激素。

A Comparative Analysis in 420 Cases of Tuberculous pleurisy and 14 Cases of Carcinomatous pleurisy

A comparative analysis was dome in 420 cases of tuberculous pleurisy and 14 cases of carcino- matous pleurisy.It revealed that the characteristics of Taberculous Plearisy are as follows:patients are often under 40 years old.The illness starts acutly.The primary clinic shows are:Fever,lacking in strength,chest-pain,cough, little effusion.Positive OT-test and evident effect to anti-TB.The carcinomatous pleurisy shows:o- ften above 50 years old,progressive dyspnea,chest-pain,middle effu- sion which often developed in the right-side,negative OT-test,no response to Anti-TB.
