Ⅰ期非小细胞肺癌VEGF的表达与预后关系

摘    要:目的:探讨Ⅰ期非小细胞肺癌(NSCLC)VEGF的表达及对预后判断的价值。方法:应用免疫组化法检测70例Ⅰ期NSCLCVEGF的表达,并用CD34标记微血管密度。结果:(1)Ⅰ期NSCLC组织中VEGF高表达41例(58.6%),低表达29例(41.4%),明显高于正常肺组织(P=0.001)。(2)VEGF的表达与肺癌组织的MVD密切相关,随着VEGF表达强度的增加,MVD呈升高趋势,VEGF高表达组MVD值为36.39±19.78,低表达组MVD值为21.67±14.76,两组MVD值差异显著(P=0.01)。(3)患者的5年生存率比较,VEGF高表达组为77.5%,低表达组为40.0%,两组差异显著(P=0.001)。(4)多因素Cox模型分析结果显示,肺癌组织VEGF表达情况为Ⅰ期NSCLC的独立预后因素(RR=3.51,P=0.007)。结论:检测VEGF的表达,有助于预测Ⅰ期NSCLC患者的预后,为术后辅助治疗提供参考依据。

Abstract:Objective:To investigate the relationship between VEGF expression, angiogenesis and prognosis on stage- Ⅰ nonsmall cell lung cancer(NSCLC). Methods:The expression of VEGF, MVD marked by CD34 in NSCLC were detected in 70 stage- Ⅰ NSCLC and 30 normal lung tissues as control by immunohistochemical method. Results:High VEGF expression was seen in 41 patients, low VEGF expression was seen in 29 patients. Expression of VEGF was significantly higher in cancer tissues than that in normal lung tissues; There was significantly difference of MVD between high VEGF expresson lung cancer (36. 39 ± 19.78 ) and low VEGF expresson lung cancer(21.67 ± 14. 76) ; The 5-year survival rate in patients with high VEGF expression (40. 0% ) was much lower than that in patients with low VEGF expression ( 77. 5% ), and VEGF expression was a independent prognostic factor in stage- Ⅰ NSCLC( RR = 3.51 ,P =0. 007 ). Conclusion: Detection of VEGF expression may help to predict the prognosis of patients with stage- Ⅰ NSCLC and guide the postoperation adjuvent therapy.
