157例丙型肝炎流行病学及临床特点研究

目的 了解丙型肝炎的流行病学及临床特点,为降低丙型肝炎的发病率,及时诊断、治疗和预防提供指导意义。方法 对157例丙型肝炎病人采用回顾性调查的方式,记录其传播途径、临床表现、肝功能、病原学标记检查等,进行统计分析。结果 丙型肝炎经血传播占73.25％(115/157),无明显原因传播者占15.28％(24/157)。所确诊的急性丙型肝炎均急性起病和近期有明确血制品使用史。慢性丙型肝炎症状轻,常有反复轻度ALT异常。HCV可与其它肝炎病毒如HAV或HBV合并感染,使病情加重。结论 丙型肝炎经血传播是首要途径,但无明显原因传播者也不容忽视。急性丙型肝炎一般不易诊断,除非有较明显的症状和病前1～6个月有明确血制品使用史。丙型肝炎易慢性化,且慢性丙型肝炎症状轻,常有反复轻度ALT异常,合并其它肝炎病毒如HAV或HBV可使病情加重、病程缩短、预后不良。

Epidemiological and clinical features of one hundred and fifty-seven patients with hepatitis C

Objective To investigate the epidemiological and clinical features of patients with hepatitis C. Methods One hundred and fifty-seven inpatients with hepatitis C were retrospectively analyzed for their transmission route, clinical manifestation, liver function test and serum viral markers. Results 73.25 % (115/157)of patients had blood transfusion history with 15.28% without definite infection origin. Every patients diagnosed as acute hepatitis had blood transfusion history and presented as acute onset. Patients with chronic hepatitis C were with mild clinical symptoms with mild elevated aminotransaminase level. Our patients with hepatitis C were always coinfected with other hepatotropic virus such as hepatitis A and/or hepatitis B, leading to the severe clinical process. Conclusion Most patients with hepatitis C were infected by blood transfusion and we should take others without definite transmission route into consideration. Patients with chronic hepatitis C tends to have recurred mild elevated aminotransaminase levels and sometimes were coinfected with hepatitis A and hepatitis B viruses.
