髋臼骨折治疗方法的选择

目的总结髋臼骨折治疗经验，进一步提高疗效。方法1999-2004年对无移位的16例和移位的3例髋臼骨折进行保守治疗，对有移位的29例髋臼骨折及时切开复位，分别选用松质骨螺钉，可吸收螺钉及重建钢板内固定治疗，术后早期行CPM锻炼。结果48例全部随访时间6～52个月，平均26个月。手术复位效果按malta影像学评分，解剖复位15例，满意复位10例，一般复位4例。临床效果评估按malta临床评分，优14例，良13例，可2例。保守治疗19例中，骨折无明显移位的16例，临床效果良好，3例骨折移位明显，未手术治疗，效果差。结论对有移位的髋臼骨折，只要病情允许，应尽早选择合适的手术入路，精确的骨折复位，有效的内固定，早期不负重功能锻炼，是获得良好治疗效果的关键。

Taking different treatments in types of acetabulum fractures.

Objective By treatments of these cases of acetabulum fractures experienced for promoting the effects of others better in future.Methods From 1999 to 2004 the total 48 cases of acetabulum fracture had been deal with separately 19 cases(16 novmoved cases and 3 moved cases) in conservative treatment and 29 moved cases in open-reduction with portico tic bone screws and absorbable screw and reconstraction steel-in-fixed and CPM exercise in early stage after operation.Results All these 48 cases had been followed from 6 to 52 months,average 28 months.According to x-ray standard of the Matta's,Completely 15 cases.satisfyingly 10 cases and normally 4 cases.As estimation of clinical treatment results,the best 14 cases,the better 13 cases and the good 2 cases.in the 19 conserved cases,16 non moved cases in excellent results,but 3 moved cases in unsatisfied.Conclusion For every case of acetabulum Fracture with displaced,so long as the patient's condition permitted,taking right access of operation as soon as possible.Replaced exactly and in-fixed Firmly and unloaded education in early stage after operation is the key getting perfect results.
