肝血管瘤的介入栓塞治疗

目的评价经肝动脉途径介入栓塞治疗肝血管瘤的临床应用及疗效。方法经导管动脉栓塞治疗肝血管瘤患者18例，主要病灶瘤体最小3.5cm×3.3cm，最大10．0cm×8．0cem，采用Seldinger技术经股动脉穿刺插管，行选择性肝动脉及肠系膜上动脉造影，将导管超选择性插管至肿瘤供血动脉后注入平阳霉素碘油乳剂，必要时用明胶海绵碎粒栓塞肿瘤供血动脉主干。所有患者均术后随访3个月～2年。结果经栓塞治疗后所有瘤体均有不同程度缩小，碘油沉积良好，术后无严重并发症如胆管损毁性病变发生。结论用平阳霉素碘油乳剂动脉栓塞治疗肝血管瘤有效且并发症少，是治疗肝血管瘤的重要手段。

Transcatheter Arterial Embolization in the Treatment of Liver Hemangioma

Objective To evaluate the clinical application and therapeutic effect of transcatheter arterial embolization in treatment of liver hemangioma. Methods Eighteen patients with liver hemangioma were treated by transcatheter arterial embolization. The sizes of main tumor were from 3.5cm × 3.3cm to 10.0cm × 8.0cm. Puncturing femoral artery with sseldinger＇s technology,hepatic artery and superior mesenterie artery angiography were applied,then superselective arterial embolization with pingyangmycin lipiodol emulsion. The supplying artery of the tumors were embolized by gelfoam particles.The cases were followed-up for three months to two years after the therapy. Results All of the tumors shrank to different degrees after the embolic therapy. The iodized oil deposited well. No serious complicated diseases such as destructive biliary damage appeared. Conclusion The transcatheter arterial embolization with pingyangmycin lipiodol emulsion to treat liver hemangioma is effective and with fewer complications, and should be the important choice of treatment for most cases.
