手足口病重症化危险因素的Meta分析

目的 探讨重症手足口病发病的主要危险因素,为今后的防治提供科学依据。方法 利用Meta分析方法对2008年至今国内外已发表的关于重症手足口病危险因素的病例对照研究进行定量综合分析。结果 共纳入16篇文献,病例3 147例,对照6 163例,有意义因素的OR值分别为:年龄<3岁(2.10)、EV71阳性(3.39)、散居儿童(1.87)、血糖>9mmol/L(21.17)、首诊医院级别(3.17)、体温>39℃且超过3d(5.93)、WBC>12×109/L(3.83)和首诊非手足口病(3.91)。结论 低龄婴幼儿、EV71阳性、持续高热、白细胞/血糖升高、首诊医院级别和首诊非手足口病是重症手足口病的危险因素。而出诊天数、现住址为农村、早产儿和流动人口与重症手足口病无关联。

Meta-analysis of the risk factors on severe hand-foot-mouth disease

Objective To provide scientific evidence for the prevention and treatment of severe hand-foot-mouth disease through studying the main risk factors. Methods Meta-analysis was performed to quantitatively review case-control study of risk factors for the severe hand-foot-mouth disease since 2008. Results A total of 16 papers involving 3147 cases and 6163 controls were included in the review.The pooled OR values of partially significant factors:age <3 years (2.10),EV71 (3.39),scattered children (1.87),GLU>9mmol/L (21.17),low level of newly diagnosed hospital (3.17),T>39℃ and more than 3 days (6.10),WBC>12×109/L (3.83),diagnosed or not during first visit (3.91). Conclusion The risk factors of severe hand-foot-mouth disease were scattered children,EV71,high fever duration,hyperglycemia,leukocytosis,level or diagnosis of newly diagnosed hospital.There was no statistical correlation between rash days,rural residence,premature,floating population and severe hand-foot-mouth disease.
