血浆置换术治疗重型病毒性肝炎的临床研究

目的探讨人工肝支持系统中血浆置换术治疗重型病毒性肝炎(重型肝炎)的临床应用效果。方法对57例重型肝炎患者进行血浆置换术治疗,观察治疗前后肝功能、电解质、血氨、凝血酶原时间等变化,比较重型肝炎不同分期应用血浆置换术的治疗效果。结果所有患者肝功能有明显改善,肾功能各项指标较治疗前有明显下降,凝血酶原时间明显缩短(P均<0.01)。早、中、晚期的治愈好转率分别为88.5%(23/26),66.7%(12/18),23.1%(3/13)。结论血浆置换术治疗重型肝炎疗效肯定,在早、中期治疗效果好。

Clinical efficacy of the plasma exchange in treatment of severe viral hepatitis

Objective To evaluate the clinical efficacy of plasma exchange of artificial liver surpport system in treatment of severe viral hepatitis (severe hepatitis). Methods 57 patients with severe hepatitis were treated with plasma exchange. The liver function, electrolyte, blood ammonia and prothrombin time changes before and after treatment were observed and a comparison to the therapeutic efficacy of the plasma exchange between different stages of severe hepatitis was taken. Results The liver function in all patients can be improved, every indexes of kidney function decreased significantly in comparison to those before treatment, the prothrombin time shortened, all P<0.01. The healing rates of early, middle and advanced stages were 88.5% (23/26), 66.7% (12/18), 23.1% (3/13), respectively. Conclusion Plasma exchange is effective in treatment of severe hepatitis, in early and middle stages can obtain a better results.
