单侧肺通气对内源性呼气末正压的影响研究

目的分析单肺通气对内源性呼气末正压(PEEPi)的影响。方法将53例食管癌手术患者双腔支气管插管后分别进行双肺和单肺机械通气,统计PEEPi发生情况。结果双肺通气时发生内源性呼气末正压11例,发生率为20.7%,单肺通气时发生内源性呼气末正压23例,发生率为43.4%,两组比较差异有统计学意义(P<0.05)。结论在机械通气过程中应根据平台压等指标对患者的呼吸参数进行调整,可以减少内源性呼气末正压的影响。
