PCR检测降钙素基因高甲基化在监测慢性髓细胞白血病恶化中的价值

摘    要:目的观察慢性髓细胞白血病(CML)病情恶化与降钙素(CT)基因高甲基化程度之间的关系。方法用内切酶酶切和PCR技术研究45例CML患者(其中慢性期33例,加速期2例,急变期10例)的CT基因高甲基化程度。CT基因未高甲基化,则能被相应内切酶切断,无法扩增出特异DNA片断,否则,能扩增出特异DNA片断,据此推断DNA的甲基化程度。结果3例慢性期,2例加速期及8例急变期患者呈现降钙素基因高甲基化;急变期组(包括加速期)甲基化率明显高于慢性期组,两组有显著性差别(P<0.01);1例在慢性期呈现CT基因高甲基化者于10个月后发生急粒变。结论该方法有助于监测CML的恶化

Abstract:Objective Observing the relation between the worsening of chronic myelogenous leukemia(CML) and the hypermethylation in the calcitonin(CT) gene. Methods The author combined the endonuclease digestion with polymerase chain reacton to study hypermethylation in the CT gene of 45 patients with CML,including 33 cases during chronic phase,2 cases during accelerated phase and 10 cases during blast crisis. If CT gene has not hypermethylation,CT gene can be digested and specific DNA segment cannot be developed,otherwise,specific DNA segment can be developed. Therefore,CT gene hypermethylation can be determined. Results It was found that 3 cases during chronic phase,2 cases during accelerated phase and 8 cases during blast crisis exhibited CT gene hypermethylation. The hypermethylation in the CT gene of worsening phase group(including accelerated phase and blast crisis) was much higher than that of chronic phase group(P<0 01). One patient with CML during chronic phase experienced a acute transformation of the disease 10 months after finding an abnormal pattern of methylation. Conclusion the method is useful to monitor the worsening of the disease.
