电视胸腔镜手术诊治恶性胸腔积液的疗效评价

摘    要:目的 :评价电视胸腔镜手术诊治恶性胸腔积液的临床疗效。方法 :对 12 0例胸腔积液患者行电视胸腔镜胸膜活检 ,确诊为恶性病变 ,喷洒滑石粉和顺铂。结果 :12 0 /12 0例手术成功 ,随访 115例 1~ 2 4个月无胸腔积液复发。结论 :电视胸腔镜手术对胸腔积液的病因诊断有临床实用价值 ,滑石粉胸膜固定加顺铂喷洒治疗是控制恶性胸腔积液的有效方法。

Abstract:OBJECTIVE:To evaluate the therapeutic effect of video-assisted thoracoscopic surgery (VATS) in the diagnosis and treatment of malignant pleural effusion.METHODS: One hundred and twenty consecutive patients with pleural effusion were studied prospectively. These patients underwent pleural biopsy by VATS procedure and were identified as malignant diseases. All cases also underwent the pleurodesis by spraying talc powder and cisplatin to the thoracic cavity.RESULTS: Pleural effusion of all patients improved completely after operation. No recurrence was found during a follow-up of 1-24 months term.CONCLUSIONS: VATS produces good results in the diagnosis of the origin of pleural effusion. The pleurodesis procedure which spray talc powder and cisplatin to the thoracic cavity is an effective method to rein the malignant pleural effusion.
