力确刀治疗重度混合痔30例的临床疗效分析

目的探讨力确刀(Ligasure)血管闭合术治疗重度混合痔的疗效和安全性。方法将60例混合痔(内痔Ⅲ、Ⅳ度)患者按随机数字表法随机分为试验组30例(采用Ligasure血管闭合系统治疗)及对照组30例(采用经典的Milligan-Morgon手术治疗),记录并比较两组患者手术时间、术中出血量;术后第一次排便疼痛指数(VAS评分法)、住院时间、创面愈合时间;肛门狭窄情况及尿潴留发生例数。结果在术中出血、手术时间、住院时间、术后第一次排便疼痛指数方面,试验组优于对照组,差异有统计学意义(P<0.05);而在肛门狭窄情况、尿潴留及术后创面完全恢复时间方面,两组差异无统计学意义(P>0.05)。结论与传统手术相比,Ligasure血管闭合术治疗重度混合痔具有术中出血少、手术时间短、术后疼痛轻、住院时间短等优势。

The analysis of clinical effect of ligasure hemorrhoidectomy on patients with severe hemorrhoids

Objective to analyze the effect and security of ligasure hemorrhoidectomy in treating patients with severe hemorrhoids.Methods 60patients were randomly divided into experiment group(by ligasure) and control group(conventional) by random number form,30cases in each group.Parameters including operating time,intraoperative blood loss,hospital stays,healing time,and pain index at the first time of defecation(VAS),anal stenosis and uroschesis of each group were recorded and compared.Results experiment group is superior to control group in terms of operation time,intraoperative blood loss,pain index at the first time of defecation and hospital stays(P <0.05);There is no significant difference of healing time,anal stenosis and uroschesis between two group(P >0.05).Conclusion compared with conventional hemorrhoidectomy,ligasure hemorrhoidectomy has advantage in terms of operative time,intraoperative blood loss,and pain index at the first time of defecation and hospital stays,but its long term effect need to be observed in the future.
