交锁髓内钉治疗胫骨骨折的临床观察

目的：探讨交锁髓内钉治疗胫骨骨折的临床效果和安全性。方法：将80例患者随机分为观察组和对照组各40例，观察组采用交锁髓内钉治疗，对照组采用加压钢板治疗．分析对比两组患者的临床疗效。结果：全部病例均获的6～12个月的随访，平均随访83±1．4个月。观察组患者骨折均获得骨性愈合，平均愈合时间为11±1．8周，愈合优良率为95．00％，无感染、断钉等并发症；对照组37患者骨折获得骨性愈合，平均愈合时间为15±2．1周，愈合优良率为72．50％，4例患者发生感染；二者临床疗效比较差异存在显著性（P〈0．05）。结论：采用交锁髓内钉治疗胫骨骨折，操作简单，局部血供破坏小，固定牢固，骨折愈合优良率率高，值得临床推广应用。

Clinical effect of treatment for shinbone fracture with interlocking intramedullary nail

Objective： To explore the clinical curative effect and safety of interlocking intramedullary nail for treatment of shinbone fracture. Methods：80 cases patient were divided randomly into the intervention group （40 cases） and the control group （40 cases）,the intervention group was treated with interlocking intramedullary nail, while the control group was treated with cmnpression plate, and then compared the effect of them. Results ： All cases were follow up for 6 ± 12 months, 8.3± 1.4 months on average. All cases achieved solid bone union, a mean healing time of 11 ＋ 1.8 weeks, the ＂good＂ and ＂excellent＂ rate is 95%, there were no complication in the intervention group ; 37 cases achieved solid bone union,a mean healing time of 15 ＋ 2.1 weeks,the ＂good＂ and ＂excellent＂ rate is 72.50% ,4 cases of infection;That the former were better than the latter （P 〈 0.05）. Conclusion ： The interlocking intramedullary nail for treatment of shinbone fracture is simple operation, less trauma on regional blood supply, stable and reliable fixation, more effective, and hence is worthy of being recommended in clini- cal practice.
