摘    要:目的 探讨早期糖尿病肾脏疾病(diabetic kidney disease,DKD)患者肾动脉血流阻力指数(renal artery blood resistive index,RI)与血清超敏C反应蛋白(high sensitive C-reactive protein,hs-CRP)的相关性.方法 选择2013年3月至8月在我院内分泌科住院的2型糖尿病(type 2 diabetes mellitus,T2DM)患者60例,依据24 h尿白蛋白排泄率(urinary albumin ejections rate,UAER)的不同将60例患者分为2组,正常白蛋白尿组(Ⅰ组)30例,微量白蛋白尿组(Ⅱ组)30例;另设我院门诊健康体检者30名为健康对照组(NC组).所有纳入对象均行血清hs-CRP及空腹血糖(fasting blood-glucose,FBG)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、三酰甘油(triglycerides,TG)、血总胆固醇(total cholesterol,TC)、尿素氮(BUN)、血肌酐(SCr)检测,同时应用彩色多普勒超声诊断仪对各组肾主动脉(main renal artery,MRA)、段动脉(stenotic renal artery,SRA)、叶间动脉(interlobar renal artery,IRA)、收缩期峰值流速(peak systolic velocity,PSV)、舒张期末血流速度(end diastolic velocity,EDV)及肾动脉血流阻力指数(renal artery blood resistive index,RI)进行测定,将所有检测结果进行统计学分析.结果 Ⅰ组、Ⅱ组患者的血清hs-CRP、FBG、TG、BUN、SCr水平均明显高于NC组,差异有统计学意义(P<0.05,P<0.01),且血清hs-CRP与各级肾动脉的EDV呈负相关(r=-0.447、-0.462、-0.472,P<0.01),与RI呈正相关(r=0.603、0.611、0.632,P<0.01).结论 早期DKD患者RI与血清hs-CRP水平相关,对于早期诊断及进行干预治疗提供临床参考价值.

Abstract:Objective To explore the correlation between renal artery blood resistive index and serum high sensitive C-reactive protein (hs-CRP) in early stage diabetic kidney disease. Meth叫s Sixty patients with type 2 diabetic mellitus (T2DM) were chosen from Endocrinology Departmentof the First Affiliated Hospital, Shi He Zi University from March to August 2013, then randomly divided into two groups according to Urinary Albumin Excretion Rate (UAER) : group I ,normalalbuminuria, and group II ,microalbuminuria.Meanwhile, 30 health examination volunteers from outpatient department as normal control (NC) group. hs-CRP, fasting blood glucose (FBG) ,high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), triglycerides (TG), total cholesterol (TC) ,BUNand SCr were determined. ColorDopplerultrasonography was used to measure peak systolic velocity (PSV) ,enddiastolic velocity (EDV) and renal artery blood resistive index (RD of main renal artery (MRA) ,stenotic renal artery (SRA) and interlobar renal artery (IRA) separately. Results hs-CRP,FBC-,TG,BUN and SCr in groups I and II were significantly higher than in NC group (P〈0. 05 or P〈0. 01), and there was a positive correlation between hs-CRP level with EDV (r = - 0. 447, - 0. 462, - 0. 472, P〈0. 01 ), and negative correlation with RI (r = 0. 603,0. 611,0. 632, P〈 0. 01 ). Conclusions In early stage diabetic kidney disease, RI is closely related to hs-CRP, which may provide clinical reference value eor early diagnosis and intervention.
