摘    要:目的 分析2型糖尿病(Type 2 diabetes mellitus,T2DM)患者骨密度(Bone mineral density,BMD)与患者年龄、空腹及餐后血糖、糖化血红蛋白、血钙、血磷、甘油三脂及总胆固醇之间的关系,并分析其临床指导价值.方法选择2011年5月至2013年5月间,在本院初次诊断T2DM并以注射胰岛素作为血糖控制方案的男性患者.采用双能X线骨密度仪(Dual energy X-ray absorption,DEXA)检测腰椎前后位(L1~4)BMD值(g/cm2),记录患者身高和体重以计算身体质量指数(Body mass index,BMI),检测患者空腹血糖、餐后2小时血糖、糖化血红蛋白A1C(Hemoglobin A1C,HbA1c)、血钙、血磷、甘油三脂及总胆固醇浓度.结果 T2DM患者腰椎前后位(L1-4)平均BMD为(1.08±0.17)g/cm2,与年龄、血钙、血磷、甘油三酯、总胆固醇、身高、体重和BMI间均无相关性.T2DM患者BMD与其空腹血糖、餐后血糖和糖化血红蛋白呈负相关,差异均具有统计学意义(r=-0.666,P<0.001;r=-0.74,P<0.001;r=-0.693,P<0.001).结论中年男性T2DM患者血糖水平越高,平均持续时间越长患者BMD越低,对于糖化血红蛋白水平高者应积极开展BMD检查以早期发现低骨量和OP患者.

Abstract:Objective To analyse the relationship of bone mineral density (BMD) with the fasting blood glucose (FBG), 2-hour post-meal blood glucose, glycosylated hemoglobin Alc (HbAlc), blood calcium and phosphorus, triglycerides (TG) and total cholesterol (TC) in male type 2 diabetes mellitues (T2DM) patients and to evaluate the clinical application. Methods The male T2DM patients who used the insulin for blood sugar control from May 2011 to May 2013 were included. The BMD of 1-4 lumbar vertebra (L1-4) were detected. The height and weight, the concentration of FBG, 2-hour post-meal blood glucose, HbAlc, blood calcium and phosphorus, TG and TC were evaluated. Results The average BMD was (1.08+0.17) g/cm2. The individual BMD has no relationship with their ages, height, weight and BMI, concentration of TG, TC, blood calcium and phosphorus. The BMD has negative correlation with the FBG, 2-hour post-meal blood glucose and HbAlc (r=-0.666, P〈0.001; r=-0.74, P〈0.001; r=-0.693, P〈0.001, respectively). Conclusion In middle-aged male T2DM patients, the higher and longer the blood sugar, the BMD is lower. The BMD testing should be given to T2DM patients who were in high level of HbAlc to screen the low bone mass and osteoporosis patients.
