全国20个省市2型糖尿病患者胰岛素心理抵抗因素及治疗中断情况的调查分析

目的了解患者胰岛素心理性抵抗因素和治疗中断因素,为提高胰岛素治疗的依从性提供科学依据。方法:对11898例2型糖尿病患者进行统一问卷调查,收集胰岛素治疗1个月及3个月的资料并对结果进行相关统计分析。结果:胰岛素心理性抵抗因素中,认为胰岛素治疗表示病情严重、对体重影响、对家庭经济影响等因素在胰岛素治疗1个月及3个月无明显变化(Kappa值>0.61),与胰岛素治疗1个月相比,3个月时认为会发生低血糖、注射疼痛的患者比例有所上升(Kappa值分别为0.41和0.47);是否参保(χ2=158.13,P<0.001)、不同教育程度(χ2=121.66,P<0.001)及不同收入水平(χ2=111.58,P<0.001)患者胰岛素治疗中断率差异具有统计学意义。结论:多数糖尿病患者存在对胰岛素治疗的误区,且不同人口学特征患者存在胰岛素治疗中断率差异,迫切需要采取有效措施加以防控或纠正。

A Survey on Psychological Insulin Resistance and Treatment Interruption in Individuals with Type 2 Diabetes in 20 Provinces in China

Objoelive To understand the main factors through analyzing the factors of psychological insulin resistance and situation of treatment interruption. Provide a scientific basis to improve the compliance of insulin therapy, so as to effectively achieve the ultimate goal of insulin treatment; Methods Adopt unified questionnaire to collect data of one and three months after insulin therapy in 11898 type 2 diabetes. And analyze the data with appropriate statistical analysis methods; Results The mental illness severity, weight change and the economic effects are highly consistent between one month and three months after insulin therapy （Kappa〉0.61）. There have statistics significance in medical insurance （%2= 158.13, P〈0.001 ）, education level x2= 121.66, P〈0.001 ） and income level （x2=111.58, P〈0.001 ）; Conchltlilm There are many misunderstandings on insulin therapy in most of diabetic patients. And there have differences on insulin treatment interruption rate for patients with different types. We can take effective measures to prevent or correct these situations.
