摘    要:目的 了解腹内压(IAP)对大鼠呼吸、循环、肾功能的影响,以及间接测压法是否具有与直接测压法同样的意义.方法 将60只SD大鼠随机分成健康对照组、IAP 1~5组(IAP值分别为5、10、15、20、25 mm Hg,1 mm Hg=0.133 kPa),每组10只大鼠.记录各组大鼠呼吸、循环、肾功能的生理指标以及IAP检测值,并将经下腔静脉及膀胱测得的间接测压值与直接测压值进行相关性分析.结果 IAP 1组大鼠与健康对照组各指标接近;IAP 2组PaO2显著降低(P<0.05),血清肌酐、尿素氮含量显著升高(P<0.05);IAP 3组、IAP 4组除呼吸频率、平均动脉压外,其余指标与健康对照组比较,差异均有统计学意义(P<0.05或P<0.01);IAP 5组PaO2、平均动脉压显著降低(P<0.01),其余指标升高(P<0.05或P<0.01).2种间接测压值与直接测压值均呈显著正相关(r值分别为0.937、0.955,P<0.01).结论 IAP对大鼠呼吸、循环、肾功能均有影响,但程度不同;经下腔静脉、膀胱间接测压法可替代损伤较大的直接测压法.

Abstract:Objective To study the effects of intra-abdominal pressure (IAP) on respiratory sys-tem, circulatory system and renal function in rats. To investigate the difference between the direct measure and indirect measure methods (via inferior vena cava and bladder) for IAP. Methods Sixty Sprague-Drawly ( SD ) rats were randomly divided into 5 groups with different lAP ( lAP value of 1-5 groups was 5,10, 15, 20, 25 mm Hg,respectively) and healthy control group, 10 rats in each group. The parameters of respiratory system, circulatory system, renal function, and lAP value were recorded. The correlation be-tween direct and indirect measurement methods was also analyzed. Results There were no significant differences in above parameters between IAPI and healthy control groups. Compared with those in healthy control group, PaO2 significantly decreased ( P < 0.05 ), SCr and BUN increased significantly in IAP2 group ( P < 0.05). Other indexes in IAP3, lAP4 groups increased ( P < 0.05 or P < 0. 01 ) except for re-spiratory frequency(RF) and MAP ( P >0.05). PaO: and MAP decreased ( P <0. 01 ), and other inde-xes increased( P <0.05 or P < 0.01 ) in IAP5 group. The values obtained from the indirect measure meth-od were positively correlated with that from the direct measure method ( r = 0.937, 0. 955, P < 0.01, re-spectively). Conclusions IAP can affect respiratory system, circulatory system and renal function in dif-ferent degrees in rats. The indirect measure method can replace the direct measure method for lAP measure with little injuries.
