膀胱移行细胞癌尿脱落细胞VEGF表达的研究

目的：研究尿脱落细胞ＶＥＧＦ表达与膀胱癌的关系，探讨其作为膀胱肿瘤诊断标记物的临床意义。方法：应用免疫组化技术检测４０例膀胱癌患者和２０例非肿瘤患者尿脱落细胞ＶＥＧＦ的表达。结果：ＶＥＧＦ在非肿瘤组尿脱落细胞无表达，而在肿瘤组尿脱落细胞表达率为４０％，联合常规ＨＥ染色检测可提高肿瘤检出率达８６．２％。复发组膀胱癌尿脱落细胞ＶＥＧＦ表达率７０％，明显高于初发组的３０％（Ｐ＜０．０５）。ＶＥＧＦ在膀胱癌的表达率与肿瘤分期有相关性，而与肿瘤分级程度无相关性。结论：检测尿脱落细胞ＶＥＧＦ表达，可作为膀胱肿瘤诊断、监测复发的良好指标，可提高尿脱落细胞检测膀胱癌的阳性率。

An study of expression of vascular endothelial growth factor in urinary cytology in transitional cell bladder carcinoma

Objective:To evaluate expression of vascular endothelial growth factor (VEGF) in urinary cytology and possibility of VEGF as a marker for diagnosis of bladder cancer. Methods: The expression of VEGF in urinary cytology by immunohistochemical staining was examined in 40 cases of transitional cell bladder carcinoma and 20 subjects with normal urothelium.Results: The positive staining of VEGF in 40 patients with bladder cancer were 40%, the expression of VEGF was negative in all controls. It was revealed that the expression of VEGF was significantly higher in recurrent lesions(70%) than in the primary cases(30%). There was also a significant difference in the expression of VEGF between those with infiltrative lesions and the superficial ones. Conclusion: The findings suggest that VEGF in urinary cytology can be used as a valuable marker for diagnosis of bladder cancer.
